It’s Time To Renew Your Certificate to Dispense
Your certificate to dispense will expire July 1st, 2012. Due to ongoing budget restraints, the online renewal option will not be offered this year for certificates to dispense. It is the Board’s hope to be able to have this option in place for next year.
Print out and complete the renewal form. Send the completed form to our office with the required fee. (money order, cashier’s check or personal check, made payable to AZND Board) Form

Pursuant to A.R.S. §38-431.02(A)(1)(a), the Arizona Naturopathic Physicians Medical Board hereby states that all notices of the meetings of the Arizona Naturopathic Physicians Medical Board and any of its committees and subcommittees will be posted at the office of the Arizona Naturopathic Physicians Medical Board at 1400 W. Washington Phoenix Arizona 85007. This location is open to the public Monday through Friday from 8 a.m. to 5 p.m., except for legal holidays and State mandated furlough days. The above notices for all meetings will also be posted on the Board’s website at www.aznd.gov under board meetings. Notices will indicate the date, time, and place of the meeting and will include an agenda or information concerning the manner in which the public may obtain an agenda for the meeting.
